The Centers for Medicare & Medicaid Services has released toolkits highlighting care and patient strategies from Medicare accountable care organizations. The care coordination strategies focus on Medicare patients who recently visited an emergency department or were discharged home after a hospital or ED visit; require treatment in a skilled nursing facility; or have complex medication regiments or chronic conditions affected by social determinants of health.
