Integrating the Community Voice in Care Delivery

Building on the themes shared during Community Health Improvement (CHI) Week, the webinar series on 鈥淭he Patient鈥檚 Voice鈥 features projects and programs highlighting member鈥檚 work in advancing the best possible patient-centric care by integrating the patient voice in care design and delivery. As part of the AHA鈥檚 Hospital Community Cooperative (HCC), member teams are advancing health equity and providing quality care by integrating the collective community voice in the implementation of place-based initiatives. Leaders from HCC teams Holy Name Medical Center and MultiCare Health System share their hospitals respective journeys towards providing the best screening and educational programs by integrating the voice of the communities they serve.
