To transform care delivery and improve health outcomes, health systems are identifying community-based strategies to improve access to services and provide an opportunity to better assess a patient’s social well-being.
This webinar will highlight two successful, real-world strategies that bridge hospital and community care:
Harvard Medical School’s Mobile Health Map: Discover how mobile health units are changing the landscape of care delivery. Speakers will share best practices, trends in mobile health and practical tools to measure impact and communicate value to funders, policymakers and community stakeholders.
Community Memorial Healthcare’s Ventura County Caregiver Coalition: Learn how the coalition supports caregivers through a unique collaboration among health care providers, social service agencies and community organizations. Attendees will also hear how their initiative has reduced hospitalizations and mental health strain while boosting caregiver resilience and reducing costs.
