Are You?

• A senior level leaders in health care (CEO, COO, CMO, etc.) at an AHA member hospital or health system
• Passionate about coaching, guiding and supporting emerging leaders?
• Available to serve as a mentor for the length of the nine month Fellowship (October 2018 – July 2019)

Mentors will:

To apply to be a mentor, please send your bio and contact information to the Health Care Transformation Fellowship staff at hctfellowship@aha.org by July 31, 2018.

• Attend mentor overview call/training session with the AHA (1 hour)
• Attend Fellowship webinars as available
• Provide coaching guidance for up to three Fellows (as needed)
• Lead up to four, 1 hour group coaching sessions
• Host a site visit at your organization for your assigned Fellows (optional, Fellow responsible for travel expense)
• Provide quarterly, written status updates to the AHA on Fellow progress
