The pandemic has strained hospitals, affecting caregivers’ well-being and resulting in many leaving their hospital jobs. The AHA has been collecting resources to assist organizations in their efforts to plan for the future post-COVID-19 workforce.</em></p><p><a href="/aha-center-health-innovation-market-scan/2023-02-28-5-imperatives-scale-virtual-nursing-program">As discussed by Sylvain Trepanier, R.N., chief nursing officer at Providence</a>, “Virtual nursing was introduced a couple of decades ago, but it’s getting a fresh look thanks in large part to its successful use during the height of the pandemic. Today, leaders at many health systems are piloting technologies and expanding programs as they work to systematically scale this model.”</p><p>Nursing leaders have identified five essential characteristics for successful virtual nursing programs:<br>1. Be inclusive from the start.<br>2. Allow time for relationship building.<br>3. Anticipate technological difficulties.<br>4. Don’t be afraid to change workflows.<br>5. The Centers for Medicare & Medicaid Services waived certain regulatory requirements and Congress provided significant legislative support to ensure hospitals and health systems could manage the numerous challenges facing them, including by increased virtual care options. These swift actions provided hospitals and health systems with critical flexibilities to care for patients during what has been a prolonged and unpredictable pandemic. We appreciate the introduction of this bill, which proposes many changes that would allow for increased patient access to telehealth services and use by hospitals and other providers, while removing barriers to adoption.</p> <p>Patients across geographies and settings, including both rural and urban areas, have benefited from increased access and improved convenience provided by telehealth services. Specifically, we strongly support the provision in the legislation which would permanently remove the geographic restrictions that currently limit where patients can access telehealth services and would add homes and other clinically appropriate sites as eligible originating site locations. We would encourage consideration of simply eliminating originating site restrictions altogether. Doing so would ensure that all Medicare beneficiaries can access services regardless of where they and their providers are physically located.</p> <p>Behavioral health is one specialty area that has seen sustained growth in telehealth utilization. In fact, prior to the pandemic, telehealth visits accounted for less than 1% of behavioral health visits. During the pandemic, they peaked at about 40% of all behavioral health visits and have been sustained at around 36%.<sup>1</sup> Geographically dispersed patients have benefited from increased access to behavioral health services provided through telehealth, especially in areas that may have provider shortages and in person visits are not possible. As a result, we appreciate the proposed removal of the requirements that a patient must receive an in-person evaluation six months before they can initiate behavioral telehealth treatment and also must have an in-person visit annually thereafter.</p> <p>Additionally, the AHA supports allowing rural health clinics and federally qualified health centers to serve as distant sites, so that these facilities may use the providers at their own sites to offer care to patients, ensuring patients remain connected to their primary providers. The AHA also supports allowing critical access hospitals (CAHs) the same ability to offer and bill for telehealth services and would encourage consideration of adding language to include CAHs as eligible distant sites.</p> <p>We also appreciate the ability to waive restrictions on the use of telehealth during national and public health emergencies and support improving Medicare’s process for coverage of telehealth services given the positive impact of improving patient’s access to care.</p> <p>Hospitals, health systems, providers and patients have seen the benefits and potential for telehealth to increase access and transform care delivery. Program organizers will work this year with public health departments to expand the program to 100,000 people in vulnerable communities. Health care leaders, executives from health care startups and others who spoke at the conference shared ways that provider organizations can reestablish connections with patients.</p> <h2><span>3 Ways to Reconnect with Patients</span></h2> <h3><span>1</span> <span>|</span> Create meaningful change in patients’ daily lives.</h3> <p>Leaders from <a href="https://www.cityblock.com/" target="_blank" title="Cityblock Health homepage">Cityblock Health</a>, a provider group that serves low-income populations, and <a href="https://grapevinehealth.com/" target="_blank" title="Grapevine Health homepage">Grapevine Health</a>, a media company that works to improve health literacy, said outreach and engagement are just part of the puzzle.</p> <p>Lisa Fitzpatrick, M.D., Grapevine’s CEO, is attempting to bridge a gap at one of the most fundamental levels — health literacy. Grapevine is working to deliver relevant, reliable and relatable health information to communities.</p> <p>Fitzpatrick has seen firsthand how many patients aren’t getting the information they need. She moved from her luxury apartment in Washington, D.C., to one of the city’s poorest neighborhoods, where health outcomes and health literacy lag. Some samplings of things she heard on the streets in her neighborhood include: “I wasn’t sick before I went to the doctor,” and, “Is cancer contagious?”</p> <p>More pressing, one neighbor didn’t fully understand how his heart condition could lead to his being out of breath. What’s she’s learned in growing Grapevine is that the message and messenger matter. People want to hear from others who look like them and have shared experiences. Information must be authentic and developed in a manner that suits that person’s needs.</p> <p>Toyin Ajayi, M.D., Cityblock’s CEO, said her group has been successful because it builds engagement with patients.</p> <p>She pointed to Cityblock programs that help pregnant women navigate their health care journey during and after delivery; and working across the care continuum to ensure real-time handoffs for behavioral health patients so they don’t fall through the cracks. It can result in more timely data coming in from internet-connected devices and may level the playing field for competitors across health care. But connectivity alone won’t solve the problems, he said. Data for data’s sake won’t result in better outcomes. What’s needed are solutions that translate and synthesize information.</p> <p>Tessema said tools like artificial intelligence and machine learning can exacerbate health inequities. “We need more transparency on where the data came from and we need to generate more data points,” he said.</p> <h4><span>Key Takeaway</span></h4> <p><em>The field must solve for the issue of biases that exist in data if it is going to realize the full potential of digital tools.</em></p> <h3><span>3</span> <span>|</span> Use technology to build a sustainable workforce and improve patient care.</h3> <p>Roberta Schwartz, executive vice president and chief innovation officer at <a href="https://www.houstonmethodist.org/" target="_blank" title="Houston Methodist homepage">Houston Methodist</a>, explained how <a href="https://www.oliverwyman.com/our-expertise/perspectives/health/2022/sep/innovation-is-the-only-way-out.html" target="_blank" title="Oliver Wyman: Innovation Is the Only Way Out">creative use of technology</a> is helping Houston Methodist build a sustainable workforce.</p> <p>The first step, she said, was throwing away the regular playbook of retention bonuses and breaking the bank on travel nurses. “Innovation is the only way out,” she said.</p> <p>Some examples: deploying a telenursing platform that relies on a centralized team of nurses linked to iPads in patient rooms to conduct intake and patient discharge. The program cut 30 minutes out of the time it takes to do those processes and freed bedside nurses to return to patient care.</p> <p>They also use the <a href="https://biointellisense.com/#:~:text=The%20Award%2DWinning%20BioButton%C2%AE%20Medical%20Grade%20Wearable%20Device%20for%20Continuous%20Remote%20Monitoring" target="_blank" title="BioIntelliSense: BioButton copy on homepage">BioButton</a>, a wearable device, to track patient vitals in real time, eliminating the need for nurses to continually wake patients at night. And chatbots and natural language processing were deployed during COVID-19 vaccination events to automate and streamline scheduling.
