The agency said the individual had conjunctivitis and no other symptoms. The survivorship team at the hospital in Portland, Maine, includes a nurse practitioner, nutritionist, physical therapist and social worker. Access Model, which seeks to increase access to treatments for Medicaid enrollees with rare and severe diseases, will initially focus on sickle cell disease, the Centers for Medicare & Medicaid The genetic blood disorder affects over 100,000 Americans, most of whom are Black and at least half of whom are enrolled in Medicaid. A total of 212 residents in 25 U.S. states and jurisdictions have been identified who might be at risk of fungal meningitis because they received epidural anesthesia at clinics in Matamoros, Mexico, during cosmetic procedures. The CDC is urging all patients, including those without symptoms, who underwent medical or surgical procedures under epidural anesthesia at River Side Surgical Center or Clinica K-3 in Matamoros, Mexico, since Jan. 1, 2023, should be evaluated for fungal meningitis as soon as possible. Health care providers should immediately report possible fungal meningitis cases possibly related to this outbreak to their state or local health department.</p> Fri, 02 Jun 2023 14:13:53 -0500 Disease Management Task force recommends biennial mammograms starting at 40 /news/headline/2023-05-10-task-force-recommends-biennial-mammograms-starting-40 <p>The U.S. Preventive Services Task Force yesterday released for public comment through June 5 a draft <a href="https://uspreventiveservicestaskforce.org/uspstf/sites/default/files/file/supporting_documents/breast-cancer-screening-draft-rec-bulletin.pdf" target="_blank">recommendation</a> that all women get screened for breast cancer every other year starting at age 40. The panel previously recommended that women in their 40s make an individual decision about when to start screening based on their health history and preferences. The draft recommendation applies to women at average risk of breast cancer, including people with a family history of breast cancer and people who have other risk factors, such as having dense breasts. Largely due to its private management organizational structure, the hospital is able to innovate, respond nimbly to patient needs, invest in cutting-edge technology, and maintain high-quality standards. MLK Community Healthcare has earned multiple awards and distinctions for its commitment to quality care, including a HIMMS level 7 rating for its use of health information technology and a five-star rating for quality from the Centers for Medicare & Medicaid Services (CMS). Notably, only 6.4% of hospitals in the nation are at HIMMS 7. In addition, only 13% of all hospitals receive a CMS five-star rating; it is even more rare among hospitals whose patient populations are predominantly covered by Medicaid or Medi-Cal.</p> <h2><span>Patient–Payer Mix</span></h2> <p>As a Metropolitan Anchor Hospital (MAH), MLK Community Healthcare continues to be guided by the belief that all patients – no matter their insurance status – deserve access to high-quality primary and specialty care services. MLK Community Healthcare is located in federally designated Medically Underserved and Healthcare Professional Shortage Areas, and it experiences a challenging patient-payer mix. Ninety-six percent of MLK Community Healthcare patients are publicly insured or uninsured, placing an extreme burden on the finances of a health system unable to crosss-ubsidize with commercial insurance.</p> <p>MLK Community Healthcare serves one of the most diverse communities in the state—94% of its patients in 2022 were Black or Brown. Decades of systemic racism and neglect are reflected in the community’s disproportionately high health and social needs. For example, life expectancy in MLK Community Healthcare’s service area is ten years lower than the rest of California and the median per capita income is $16,927—less than half of the state average.</p> <p>Chronic health conditions are disproportionately prevalent in the MLK Community Healthcare service area. The rate of diabetes is three times that of the rest of the state, while mortality from diabetes is 72% higher. Amputations and diabetic wound care are among the most commonly performed procedures at the hospital. In 2022, the prevalence of substance use disorder among MLK Community Healthcare patients was three times the state rate, and the number of patients with heart failure was twice as high as the statewide average. This high prevalence of chronic conditions means that MLK Community Healthcare’s patients are at an increased risk for complications, including severe symptoms from COVID-19. During the height of the pandemic, MLK Community Healthcare saw more COVID-19 patients than hospitals three to four times its size throughout Los Angeles County and was included by Becker’s Review among the 14 most highly impacted hospitals for COVID-19 in the nation.</p> <h2><span>MLK Community Healthcare's Unique Approach to Care</span></h2> <p>MLK Community Healthcare offers numerous programs to promote equal access to care and address community members’ basic and emergent health care needs:</p> <h3><span>Emergency Department</span></h3> <p>Given the lack of health care resources outside the walls of MLK Community Healthcare, community members often come to the Emergency Department to receive basic services. Even women seeking routine pregnancy-related services are cared for—and given follow-up appointments— to ensure their health and recovery. The hospital is also renowned for its triage services. Due to the use of MLK Community Healthcare’s ED for both urgent and primary care services, a department designed for 40,000 patients each year has stretched its resources to serve up to 116,000 patients annually. Yet, this has not compromised its performance. Due to process innovations, wait times are less than two hours and the Left Without Being Seen rate is the lowest among all hospitals in the area.</p> <h3><span>Investment in a Service-Oriented Health Care Workforce</span></h3> <p>Wealthy communities in Los Angeles have ten times the number of physicians located in the community than that which MLK Community Healthcare serves. To bridge this gap, MLK Community Healthcare secured over $20 million in private philanthropy to subsidize doctors’ and nurses’ salaries – helping attract a strong, service-oriented workforce, including specialists in the chronic health conditions that are disproportionately prevalent among MLK Community Healthcare’s patients.</p> <h3><span>Disease Management</span></h3> <p>For many MLK Community Healthcare patients living with multiple chronic conditions, health care services must extend beyond primary care. That’s why MLK Community Healthcare has invested in the creation of a Diabetes Center of Excellence, implementing a range of services designed to prevent avoidable ED visits and slow the progression of disease through self-management, specialized education, peer support, and a fresh produce program for food insecure patients.</p> <h3><span>Integrated Behavioral Health</span></h3> <p>An average of 10,000 patients come to the MLK Community Healthcare ED with behavioral health needs each year. With philanthropic support, MLK Community Healthcare tested an innovative pilot program integrating medical and behavioral health care in the emergency and inpatient environment with long-term follow-up treatment in the outpatient environment. poliovirus, the Centers for Disease Control and Prevention <a href="https://www.cdc.gov/media/releases/2022/s0913-polio.html">announced</a> yesterday. Genetic sequences from the virus in the patient and wastewater specimens have been linked to wastewater samples in Jerusalem and London, indicating community transmission, CDC said.<br />  <br /> Thirty other countries have circulating vaccine-derived poliovirus, which is not caused by the polio vaccine but occurs when local immunity to poliovirus is low enough to allow prolonged transmission of the original weakened virus in the oral polio vaccine. Oral polio vaccine has not been used or licensed in the U.S. since 2000 but continues to be used in some countries. N.Y. Governor Kathy Hochul last week declared a state emergency to help expand vaccination efforts and surveillance. <br />  <br /> “Polio vaccination is the safest and best way to fight this debilitating disease and it is imperative that people in these communities who are unvaccinated get up to date on polio vaccination right away,” said Dr. José R. Through its <a href="https://healthcare.utah.edu/locations/intensive-outpatient-clinic/" target="_blank" title="University of Utah Health: Intensive Outpatient Clinic landing page">intensive outpatient care program</a> (IOCP), UofU Health is taking a deeper look at patients with multiple, complex medical and behavioral health issues, notes Peter Weir, M.D., executive medical director of population health.</p> <h2>UofU’s Intensive Outpatient Clinic Model</h2> <p>UofU’s IOCP is a primary care clinic for high-risk, high-need Medicaid patients. The clinic was designed to demonstrate financial sustainability while providing comprehensive care to patients with multiple needs. It features a team of multidisciplinary clinicians including both primary care, behavioral health and substance-use treatment providers as well as social workers with experience in providing trauma-informed care. These clinicians all work together to treat both the physical, mental and social needs of a defined patient population.</p> <h2>The Benefits of an Intensive Approach to Outpatient Care</h2> <p>UofU Health established the IOCP in response to a need for innovative care models for the highest-risk and highest-cost patients. Weir, a pioneer in integrated health care, and his colleagues based their approach on other integrated clinical models that were successful in reducing hospitalizations while improving patient adherence to treatment protocols — a key factor in improving patient outcomes.</p> <p>Weir was an author of a study that was published in <a href="https://academic.oup.com/tbm/article/10/5/1187/5921055" target="_blank" title="Translational Behavioral Medicine: A systematic review of intensive outpatient care programs for high-need, high-cost patients">Translational Behavioral Medicine</a>. The study examined the effectiveness of IOCPs in achieving overarching goals of reducing emergency department visits and hospitalizations, and improving care continuity and patient outcomes.</p> <h3>The study found that IOCPs improve patient outcomes by:</h3> <ul> <li>Significantly lowering hospital admissions — with a 54% reduction of admissions at six months and a 46% reduction in admissions at 12 months.</li> <li>Lowering emergency department visits — with a 25% reduction at six months.</li> <li>Reducing length of hospital stays.</li> </ul> <h2>How Establishing an Integrated Approach Impacts Costs</h2> <p>The cost of developing a multidisciplinary care team is a perceived obstacle to creating an integrated model. However, just the opposite is true. “Our hypothesis was that we could reduce unnecessary costs by more than what we would internally cost to function. That’s what ended up working out,” Weir said in a recent <a href="https://stateofreform.com/featured/2022/03/qa-u-of-u-medical-director-discusses-clinic-successfully-addressing-sdoh/" target="_blank" title="State of Reform: Q&A: U of U medical director discusses clinic successfully addressing SDOH ">interview</a>.</p> <p>A 2019 <a href="https://jamanetwork.com/journals/jama/fullarticle/2755315" target="_blank" title="JAMA: To Control Costs Expand Managed Care and Managed Competition">Journal of the American Medical Association (JAMA) study</a> reported that large health systems can save millions annually by providing comprehensive integrated health services. A separate <a href="https://jamanetwork.com/journals/jama/fullarticle/2545685" target="_blank" title="JAMA: Association of Integrated Team-Based Care With Health Care Quality, Utilization, and Cost">JAMA study</a> reported on a 10-year study of Intermountain Healthcare’s integrated team-based care practices. The study, one of the largest of its kind, found that payments to providers in team-based practices resulted in a savings of 3.3% over traditional practices. The provider payments amounted to less than the investment costs Intermountain incurred in creating a team-based model.</p> <p>Some health systems find external funding sources to support behavioral health integration. New Jersey-based <a href="/system/files/media/file/2021/11/FINAL_mia_cs_behavioral_hackensack-meridian.pdf" target="_blank" title="Members in Action Case Study: Intregrating Physical and Behavioral Health—Hackensack Meridian Health | Hackensack, NJ">Hackensack Meridian Health</a> secured more than $5 million annually in federal and state primary care funding to develop specialized programs with integrated behavioral health services.</p> <p>An <a href="/system/files/media/file/2019/05/aha-trendwatch-behavioral-health-2019.pdf" target="_blank" title="Trendwatch: Increasing Access to Behavioral Health Care Advances Value for Patients, Providers and Communities">AHA Trendwatch report</a> offers additional examples of the cost benefits of integrating behavioral and physical care.</p> <p>In a normal fee-for-service environment, hospitals don’t have the resources to adequately address the social factors influencing health, Weir notes. “It’s a mismatch between the payment model and the care model,” he said. “We flipped the equation.
