However, these communities and their hospitals face many challenges. As the hospital field engages in its most significant transformation to date, many are fighting to survive – potentially leaving their communities at risk for losing access to health care services. Recognizing these challenges and the need for new integrated and comprehensive health care delivery and payment strategies, the AHA Board of Trustees created the Task Force on Ensuring Access in Vulnerable Communities. Comprised of 29 hospital and health system leaders and state hospital association CEOs, the task force held meetings, heard from policymakers and conducted field hearings to speak with hospital and community leaders during a 15-month period.<br><br>The Task Force work is ongoing. href="https://www.congress.gov/bill/119th-congress/house-bill/1?s=1&r=1&q=%7B%22search%22%3A%22%5C%22One+Big+Beautiful+Bill+Act%5C%22%22%7D" target="_blank" title="Congress.gov: H.R.1 - One Big Beautiful Bill Act">OBBBA; Public Law No. 119-21</a>, a sweeping package that enacts many of the administration’s legislative priorities on taxes, border security and energy. The bill includes significant policy changes to Medicaid and the Health Insurance Marketplaces.</p><p>According to the <a href="https://www.cbo.gov/publication/61570" target="_blank" title="Congressional Budget Office: Estimated Budgetary Effects of an Amendment in the Nature of a Substitute to H.R. 1, the One Big Beautiful Bill Act, Relative to CBO's January 2025 Baseline">Congressional Budget Office (CBO) score</a> of a draft version of the Senate bill, the OBBBA will lead to nearly $1 trillion in Medicaid cuts and more than 10 million people losing Medicaid and Health Insurance Marketplace coverage. A significant portion of the savings will result from new limits on the use of Medicaid provider taxes and state-directed payments (SDPs), which have historically allowed hospitals to bridge the chronic underpayments for the care they deliver to Medicaid enrollees. Each year the AHA works closely with our policy team and other key subject matter experts to refine the survey so that it aligns with key issues. Widely recognized as the most consistent and credible source of information about America’s hospitals, the survey reaches more than 5,000 hospitals and 400 health systems and yields a response rate of over 70%. There are over 1,300 hospital data points collected directly from hospitals. Data points collected include those related to facilities and services, staffing, utilization, organizational structure, social determinants of health, as well as other areas.
